Abstract

This thesis presents an analysis and evaluation of a high reliability program malfunction reporting system as a managerial aid. Interpretative regression analyses of actual high reliability hardware failure data are presented. The need to develop a form for documenting the failure event and then accurately recording the circumstances surrounding the event are highlighted. Two appendices support the research conducted on this thesis: "Procedure for Completing a Malfunction Report" and "Procedure for Completing a Malfunction Analysis".
